What you'll do

• Assess patients’ physical, functional, psychosocial, and environmental needs
• determine hospice eligibility and levels of care
• develop, document, and revise care plans
• educate patients and families about disease processes and end-of-life care
• coordinate services with the interdisciplinary team
• direct hospice aides and LPN/LVNs
• document clinical progress
• communicate changes to physicians and other personnel
• monitor cases for payer compliance
• participate in hospice training and quality improvement
• and provide on-call services as assigned.
