H2O Outreach Specialist

What you'll do

• Conduct street, shelter, and community outreach
• build rapport using trauma-informed and culturally responsive practices
• assess immediate safety, medical, and behavioral health needs
• connect individuals to services and basic needs
• screen for AHCCCS eligibility and support Medicaid enrollment
• assist with identification and legal documents
• assess housing needs and develop housing care plans
• support housing applications, transitions, and tenancy
• coordinate with providers, landlords, and community organizations
• maintain program documentation and track outcomes.

Why this role

High school diploma or GED; Microsoft Office skills; ability to obtain First Aid and CPR certification; valid driver’s license, proof of insurance, and motor vehicle report; frequent travel within the service area. Bilingual English/Spanish is desired.
