Hospice Registered Nurse (RN) Case Manager

The Hospice Registered Nurse (RN) Case Manager role is onsite in Colorado Springs, CO, with care delivered in patients’ homes or other care settings. Compensation is not specified. The role supports Choice Hospice’s care for people facing life-limiting illnesses and their families. The nurse coordinates individualized hospice care to address patients’ physical, emotional, and psychosocial needs and promote comfort and dignity. Working with an interdisciplinary team, the case manager assesses eligibility and changing needs, develops and documents care plans, guides other care staff, and supports patients and families through end-of-life care.

What you'll do

• Complete initial and ongoing comprehensive assessments
• apply hospice admission and recertification criteria
• develop, implement, and document care plans and recommend revisions
• educate patients and families about disease processes, self-care, end-of-life care, and nutrition
• determine service scope and frequency and assess caregiver capacity
• provide clinical direction to hospice aides and LPN/LVNs
• prepare clinical and progress notes
• communicate changes and outcomes to physicians and other personnel
• and perform assigned on-call responsibilities.
