Care Team Lead, Registered Nurse (RN)

The Care Team Lead, Registered Nurse (RN) role is primarily remote, serving the greater Seattle area and Eastside, with in-person client assessments and occasional training or team meetings. Compensation is based on assessments and active caseload: $150 per initial assessment, $75 per 90-day reassessment, and $50 per client per week for other client management; other support fees vary. 2nd Family provides personalized, non-medical in-home care for seniors and other adults. The nurse coordinates communication among clients, loved ones, and caregivers, and helps ensure care plans are understood and updated. The role supports consistent, client-centered service through assessments, caregiver orientation, mentorship, and ongoing case management.

What you'll do

• Conduct initial consultations and assessments and document care plans
• reassess clients every 90 days or after a change in condition
• orient assigned caregivers and assess their skills
• serve as a primary contact for clients and loved ones
• direct and mentor caregivers
• attend monthly company meetings
• occasionally support new employee orientation and in-service training
• review caregiver shift notes and follow up as needed
• respond to occasional off-duty emergency communications.
