What you'll do

• Provide case-management support as part of a multidisciplinary team
• assess health needs and connect clients with urgent, primary, and specialty care
• conduct outreach in streets, shelters, encampments, and community sites
• coordinate medication services and provide medication education
• perform histories, physicals, and assessments
• administer oral medications, injections, and wound care within scope and plan of care
• reconcile medical records
• coordinate with care teams and providers
• document assessments and plans of care
• participate in clinical reviews and quality assurance.
