• Provide community-based support consistent with individualized service plans
• help clients identify goals, strengths, needs, and barriers
• coordinate access to behavioral health services and community resources
• support independence and community integration
• monitor progress and document outcomes
• communicate concerns to the CST Team Lead and clinical professionals
• assist with crisis prevention and safety planning
• coordinate with providers and community partners
• maintain accurate, confidential records and follow applicable requirements.
