Referral Coordinator

What you'll do

• Manage incoming and outgoing referrals by verifying patient information and eligibility
• coordinate with providers, insurers, and external agencies
• maintain referral records in EHR systems or databases
• update patients on referral status and assist with appointment scheduling
• follow organizational, privacy, and insurance requirements
• collaborate to resolve referral issues and improve workflows
• prepare referral activity reports and identify trends.
