What you'll do
- Assess residents’ emotional and psychosocial needs
- develop resident-centered care plans with residents and families
- coordinate admissions paperwork, insurance authorizations, and financial assistance information
- participate in resident assessments and behavior meetings
- provide counseling or referrals
- advocate for resident rights
- lead discharge planning and coordinate transitions with outside resources
- complete progress notes and participate in quality and interdisciplinary meetings.
