What you'll do

• Provide practical support, advocacy, coordination, and assistance with daily-living needs and access to services
• participate in treatment-team meetings
• develop, implement, evaluate, and revise service-plan goals
• coordinate services and communication among providers
• support consumers and families
• encourage primary and preventive care
• document progress and complete service plans, assessments, notes, and authorizations
• assess symptoms and treatment response and adjust plans as appropriate
• provide direct individual, group, and family services
• coordinate with inpatient services and community resources
• support care plans and community education.
