What you'll do

• Coordinate resident assessment, planning, implementation, and evaluation through the MDS process
• facilitate interdisciplinary care conferences
• complete and review Minimum Data Sets
• educate and assist staff with assessment and MDS processes
• implement MDS/PPS updates and provide staff in-service
• cross-train between PPS and OBRA
• conduct resident interviews
• and respond to clinical emergencies, regulatory visits, surveys, and issues outside normal work hours as needed.
