What you'll do

• Coordinate mobile clinic schedules, logistics, supplies, and on-site patient flow
• drive mobile clinic vehicles to designated sites and maintenance locations
• conduct patient intake, eligibility screening, and enrollment
• schedule appointments and referrals and facilitate follow-up
• document in the electronic health record
• provide health education in English and Spanish
• conduct outreach and maintain community partnerships
• connect patients with social services and address access barriers.
