What you'll do

• Educate pharmacy patients on factor dosing, products, medication administration, bleed recognition, and emergency protocols
• coordinate complex cases and triage
• communicate nursing assessments to providers
• support individualized dosing plans and factor-utilization reviews
• monitor adherence, factor use, and bleed tracking
• conduct home visits and needs assessments
• facilitate prescriptions and orders
• assist with home-infusion training and troubleshooting
• coordinate appointments
• document care in the EMR.
