What you'll do

• Meet new residents upon admission and complete initial psychosocial assessments
• visit residents to follow up on concerns
• may complete some MDS assessments under supervision
• arrange needed resident purchases and clothing
• schedule dental, eye, podiatry, and audiology visits
• support residents and families during times of loss
• assist residents in gathering documents for Medicaid, Medicare, SSI, and other support services
• perform other duties as directed by the Social Services Director.
