What you'll do

• Performs initial, ongoing, and functional health status assessment as applicable to the population and or individual. Based on nursing diagnoses and collaborative problems, documents planned nursing interventions to achieve outcomes appropriate to patient needs. Evaluates and documents response to nursing interventions and achievement of outcomes at appropriately determined intervals
• as part of a multidisciplinary team, revises plan of care based on evaluative data. Actively participates in process improvement activities to achieve targeted measures of clinical quality, customer satisfaction, and financial performance.
