Transitional Case Manager (CoC)

The Transitional Case Manager (CoC) role is onsite in South Bay, FL. Pay is $19.29 per hour. The GEO Group provides correctional and community reentry services, including evidence-based rehabilitation and post-release reintegration support. The case manager works with individuals within one year of release to develop transition plans and connect them with community resources. The role supports successful reintegration through case management, treatment sessions, benefits and identification assistance, and coordination with facility and community partners.

What you'll do

• Use GEOtrack to document case notes and track program and transition needs
• develop case plans and transition plans with inmates
• connect individuals with community resources before and after release
• conduct individual and group cognitive behavioral sessions as needed
• help individuals obtain eligible benefits and identification
• coordinate with state personnel and service providers
• support resource fairs
• attend staff meetings
• compile statistics and maintain required documentation.
