The Home and Community Support Professional (HCSP) role is onsite in Greenfield, IN, with a 6 AM–10 AM Monday–Friday shift. Pay is $15–$17 per hour. Thriving Together Support Services supports individuals with disabilities or special needs in living fulfilling, independent lives. This role provides personalized daily support, helps clients with care and activities, and transports them to day services and other destinations. Accurate documentation and communication with clients, families, and care team members help maintain safe, individualized support.

What you'll do

• Provide direct care and personalized support
• assist with personal hygiene, meal preparation, mobility, and other daily living activities
• administer medications
• implement individualized support plans
• transport and accompany clients to appointments, community outings, and day service
• document care activities and progress
• communicate with clients, families, and care team members
• foster a safe, supportive environment.
