What you'll do

• Assist families with basic needs and resources
• develop service plans with measurable goals
• provide in-home services when appropriate
• visit each client at least weekly
• connect clients with community services
• maintain case notes and records
• complete referrals, social histories, and closure reports
• participate in team meetings and staff reviews
• follow up on referrals and coordinate services.
