What you'll do

• Conduct initial psychosocial, emotional, spiritual, and bereavement assessments
• help develop and implement goal-directed care plans
• participate in interdisciplinary team conferences
• document patient and family visits, calls, and referrals
• reassess needs and provide counseling as required
• provide crisis intervention and individual or family counseling when indicated
• advocate for patients and families
• coordinate with community agencies
• participate in the Bereavement Team as assigned
• follow hospice policies.
