What you'll do

• Provide direct resident care and distribute medications according to individualized care plans
• perform assessments as required
• observe and document changes in condition and communicate concerns
• record vital signs, weights, and blood sugars as assigned
• assist with daily living activities, restorative nursing techniques, and behavioral support
• encourage participation in activities
• support fall prevention
• manage medication records and pharmacy coordination
• and help orient, train, and coach caregivers.
