What you'll do

• Collaborate with providers and care-team members on patient-centered care plans
• provide nursing care and assess or triage concerns virtually, by phone, or in person
• implement and evaluate care plans
• coordinate follow-up and preventive services
• document patient information in the EHR
• advocate for patients and families
• participate in daily operations, meetings, and quality improvement
• support culturally competent care.
