What you'll do

• Conduct assigned in-person field visits
• verify participant residence and document findings
• observe and record general safety and wellness indicators
• collect household and contact information according to protocols
• use approved mobile tools and secure systems for documentation
• coordinate with supervisors, dispatch, language access, quality review, and program operations
• complete follow-up visits as required
• escalate safety concerns or reportable risks
• maintain confidential records and travel logs
• participate in training and briefings.
