The Registered Nurse (RN) is an onsite, full-time role in Hayward, CA, typically scheduled Monday through Friday with periodic Saturday hours. Tiburcio Vasquez Health Center provides accessible community care across primary care, dental, WIC, mental health, and health education services. The RN provides and coordinates professional nursing care through telemedicine and in-person services, supporting patient health and whole-person care. The role contributes to an interdisciplinary team through triage, direct care, patient education, referrals, documentation, and quality improvement.

What you'll do

• Provide nursing services including triage, care coordination, referrals, health education, and direct care such as immunizations and medication administration
• coordinate with clinical support staff
• conduct telephone and in-person triage and document assessment information
• conduct co-visits with physicians and advanced practice providers
• perform Mantoux tuberculin skin test interpretation and documentation
• address after-hours nursing notifications
• educate patients and families
• participate in quality improvement and population health initiatives
• support team members, nursing policies, staff orientation, and training
• maintain patient records in EPIC.
