What you'll do

• Assess and reassess hospice patients
• provide skilled nursing services and treatments
• implement and update patient and family care plans with the interdisciplinary group
• monitor and report changes
• educate and support patients, families, and caregivers
• supervise and support assigned clinical staff
• coordinate care
• maintain clinical documentation and follow privacy and infection-control requirements.
