What you'll do

• Educate patients and families about diagnoses, treatment options, and procedures
• identify barriers such as transportation, financial strain, insurance, language, childcare, or housing and connect patients with resources
• coordinate appointments, tests, procedures, and handoffs
• document navigation activities and outcomes
• coordinate patient care and provide nursing support within established guidelines.
