The practitioner contributes to the clinic's integrated care model, working alongside behavioral health, case management, and pharmacy support to improve community health outcomes.

What you'll do

• Manage a patient panel with 18 patients per day.
• Provide wellness, preventive care, and chronic disease management.
• Work a 4-day workweek (4x10s) with rotating Saturday coverage and light phone-based call.
