What you'll do

• Support individuals with budgeting and money management
• guide hygiene and household tasks
• engage individuals in social activities and community access
• maintain health and safety
• complete daily progress notes and quarterly reports
• follow Individualized Service Plans
• attend training, workshops, and biweekly supervision
• maintain confidentiality
• facilitate conflict resolution as needed
• attend physician appointments, SIS assessments, and annual meetings related to individual goals.
