Reentry Case Manager — Part-Time

The Reentry Case Manager role is based in Portsmouth, VA, with travel throughout the region and work on the road. Compensation is $21.68 per hour. What you'll do

• Remain assigned to youth from before release through the reentry process
• develop individual plans of care and provide support services, including educational, vocational, and employment opportunities
• prepare client visit records, progress notes, and case notes
• respond to emergencies and crisis interventions
• establish working relationships with families, court personnel, referral agencies, and service providers
• travel in own vehicle, with mileage reimbursed.
