What you'll do

• Plan, organize, and implement activity and leisure programs
• assess resident preferences and needs and develop individualized plans of care
• complete activity-related MDS assessments and documentation
• attend resident care plan meetings and document progress
• prepare and post monthly activity calendars
• monitor participation and identify risk factors or changes in condition
• plan community activities involving residents, families, staff, and community members
• coordinate volunteers and resident council meetings
• contribute to department budget planning and monthly reporting.
