In-Home HHD Dialysis Care Partner – 1:1 Client

The In-Home HHD Dialysis Care Partner – 1:1 Client role is onsite in Oklahoma City, OK. Pay is $15 per hour, and the schedule is four to five days a week. Herewith Caregivers provides training and clinical team support for a patient transitioning from clinic dialysis to home treatments. The care partner provides consistent one-on-one support to help the patient remain safe and supported during home dialysis. The role assists with treatment setup, maintains a clean and safe treatment environment, offers companionship, supports treatment timing, and reports safety concerns to the coordinating nurse.

What you'll do

• Assist with setup of NxStage or Tablo dialysis equipment
• keep the treatment area clean, organized, and safe
• provide companionship and emotional reassurance
• help maintain treatment schedules
• provide light activities-of-daily-living support as needed
• monitor safety and report concerns to the coordinating nurse
• maintain simple documentation and shift notes.
