What you'll do
- Monitor high-risk patients, support group participation, and provide redirection
- report outcomes to program leaders
- conduct specialty groups and document attendance in the electronic medical record
- track program evaluations
- communicate with patients
- monitor activity and program guidelines
- document observations, activities, and participation in patient records and shift logs
- prepare incident reports
- support the counseling team
- meet individually with patients when appropriate
- uphold organizational policies and boundaries.
