The role contributes to residents’ well-being through care-plan support, medication administration, documentation, and coordination with outside providers.

What you'll do

• Assist with resident admissions, screening, assessment, and care needs
• ensure care follows resident care plans
• administer medications and treatments according to physician orders
• coordinate care and services with outside providers, including resident physicians
• document and report care and observations, including significant changes
• and maintain working knowledge of state regulations, policies, and procedures.

Why this role

Requires graduation from an accredited practical nursing school and a current nursing license and state registration as an LPN or RN. Three years of assessment and geriatric nursing experience is preferred.
