What you'll do

• Receive and document calls about client care
• enter orders and update care plans, medication profiles, and schedules
• review interim orders
• support intake and referral processing
• assign licensed professionals for evaluation visits
• participate in case conferences and process recertification or discharge decisions
• follow up on orders and entitlement issues
• verify and requisition medical supplies
• enter non-admit information
• and review missed-visit reports and notify physicians when needed.
