Social Services Director

The Social Services Director role is onsite in Fort Wayne, IN. Compensation is $24–$28 per hour. The director supports the Social Services Department at Chateau Rehabilitation and Healthcare Center, addressing residents’ medically related emotional and social needs. The director oversees social services operations and works with residents, families, staff, and the interdisciplinary team to protect resident rights and support psychosocial well-being. Assessments, care planning, therapeutic interventions, education, and discharge coordination help meet residents’ individual needs and quality-of-life goals.

What you'll do

• Plan, organize, implement, evaluate, and direct the Social Services Department
• review policies and support regulatory compliance
• participate in quality assurance and resident-care policy work
• document psychosocial progress
• promote resident rights and psychological well-being
• complete social histories and psychosocial assessments
• contribute to interdisciplinary care plans
• provide or arrange therapeutic interventions and family education
• coordinate discharge planning and referrals
• educate staff and residents about rights, diversity, abuse prevention, and available services
• supervise assigned students.

Why this role
