What you'll do

• Provide skilled nursing care and coordinate home-based services based on assessments and physician-authorized plans
• assess and monitor patient status
• supervise Home Health Aides and CNAs
• coordinate with physicians, team members, and community resources
• educate patients and families
• document care, including OASIS/HIS/HOPE
• participate in quality initiatives
• and rotate into on-call coverage for phone triage or in-person visits.

Why this role

May begin as a Visiting RN and transition to Case Manager after assuming a half caseload. Evening and weekend on-call rotation is required.
