Behavioral Health Care Coordinator

Job overview

The Behavioral Health Care Coordinator role is onsite in Cross City, FL, with pay of $16–$18 per hour. The position is in the Managed Care department and reports to the Care Coordination Clinical Manager at North Florida Pediatrics. The coordinator helps rural patients navigate mental health and substance use treatment and connects primary care providers with behavioral health resources. The role develops care plans, coordinates check-ins and consultations, maintains patient tracking and documentation, and follows up after emergency or crisis care.

What you'll do

• Develop personalized care plans, coordinate monthly telephonic or telehealth check-ins, and guide patients through chronic care management. Help patients locate clinics, mobile outreach units, and regional behavioral health centers. Connect rural primary care providers with psychiatric consultants, emergency services, and community support teams
• arrange and guide patients through virtual consultations and therapy
• maintain secure patient tracking and compliance documentation
• and follow up after emergency room visits or crisis interventions.
