The Family Medicine Physician — Advanced Primary Care role is onsite in Springfield, OR, serving the Eugene area. The guaranteed base salary is $275,000–$300,000 per year for a two-year guarantee. Praxis Health’s Advanced Primary Care team uses a value-based model focused on access, prevention, population health, and patient relationships. The physician provides comprehensive adult primary care, including preventive care, chronic disease management, and wellness services. The role contributes to individualized care and population health in collaboration with advanced practice providers, behavioral health, clinical, operational, and population-health colleagues; a broad range of primary care procedures and women’s health services may be provided.

What you'll do

• Provide adult primary care for patients ages 18 and older
• deliver preventive care, chronic disease management, and wellness services
• contribute to population health
• collaborate with APP, behavioral health, clinical, operational, and population-health support
• may provide primary care procedures and women’s health services
• participate in very light shared call.
