What you'll do

• Meet new patients, explain services, and complete start-of-care assessments
• develop individualized care plans
• monitor progress and report condition changes
• teach or supervise household members assisting with care
• guide home care nurses, aides, and personal care staff
• review documentation and conduct supervisory visits
• oversee staff training
• ensure regulatory adherence
• prepare clinical notes and compliance records
• participate in quality reviews
• maintain community and referral relationships
• assist with organizational growth during mergers and acquisitions.
