The Registered Nurse (RN) Navigator — Cardiac Services role is onsite in Fresno, CA. Compensation is $53.09–$67.56 per hour. The role supports Community Regional’s advanced cardiac program and its high-acuity cardiac patients. The nurse serves as a consistent clinical connection for patients and families, coordinating care from referral through post-procedure follow-up. The role works with multidisciplinary teams to support continuity, safety, patient understanding, and improvement across cardiac services.

What you'll do

• Serve as clinical resource and liaison for assigned cardiac patients
• coordinate care from referral through post-procedure follow-up
• partner with cardiologists, cardiac surgeons, interventional teams, and inpatient units
• monitor progress and identify needs
• support safety, continuity, and adherence to evidence-based guidelines
• contribute to performance improvement and patient-experience initiatives.

Why this role

Requires three years of recent cardiology, pulmonology, cath lab, or care coordination nursing experience, a California RN license, BLS, and a BSN or applicable national certification.
