What you'll do

• Advocate for clients and help them access treatment, medical care, housing, food, clothing, and benefits
• support individual service plans
• provide coaching in social skills, money management, hygiene, and job readiness
• coordinate holistic case management
• provide on-the-spot counseling
• complete documentation
• and drive on program business for outreach, visits, appointments, and meetings. Provide after-hours on-call coverage on a rotational basis.
