Family Support Specialist

What you'll do

• Engage referred families and administer the Family Resilience and Opportunities for Growth assessment
• provide regular home visits and family support
• develop and update Family Service Plans
• support family goal-setting and parent education
• strengthen parent-child interactions
• conduct family screens and make referrals
• connect families with community resources
• document contacts and required information in the Family Wise Database.
