What you'll do

• Conduct intake and eligibility screening
• identify family strengths and service needs
• develop plans and support goal planning
• provide navigation, information, and referrals
• coordinate with internal programs and community providers
• attend community meetings and support information sharing
• conduct outreach
• maintain referral records
• contribute to reports and shared learning
• support implementation of FRC mobile.
