What you'll do
- Conduct admission assessments, including MDS-HC assessments, and complete required clinical documentation. Make in-home supervisory visits at least every two months
- develop and maintain individualized plans of care
- review physician summaries and coordinate medical clearances
- orient caregivers
- monitor member health and escalate changes
- document visits
- review caregiver logs
- submit required reports
- participate in audits and team conferences
- assist with referrals and discharge planning.
