What you'll do

• Build professional relationships with residents
• meet with families to support service plan goals and daily living skills
• conduct property rounds and report concerns
• document interactions and incidents
• assist transitions from intake through discharge
• protect confidential information
• mediate and support crisis resolution
• drive clients in agency vehicles
• answer phones and support reception as needed
• and maintain CPR/First Aid certification.
