Hospice Registered Nurse (Per Diem)

The Hospice Registered Nurse (Per Diem) role is onsite in South Portland, ME, providing care in patients’ homes and communities. Compensation is not specified. What you'll do

• Complete initial and ongoing patient and family assessments
• develop and revise plans of care
• provide nursing care, prescribed medications and treatments, and palliative interventions
• counsel and educate patients and caregivers
• document care and communicate changes to physicians
• coordinate with community partners and the interdisciplinary group
• participate in on-call duties as defined by policy
• supervise ancillary personnel when required.
