What you'll do

• Develop and execute a territory sales plan
• build and maintain relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, rehabilitation facilities, case managers, and discharge planners
• identify new referral sources and grow existing accounts
• conduct visits, presentations, facility rounding, and community networking
• educate partners on services, eligibility, payer requirements, and referral processes
• monitor account performance and market trends
• collaborate with internal teams to support referrals and resolve barriers
• maintain CRM records and sales reports
• meet sales activity and performance goals
• comply with HIPAA and applicable requirements.
