What you'll do

• Serve as primary RN for patient caseload
• complete assessments and determine hospice eligibility
• lead admission process
• develop individualized plans of care
• coordinate interventions with interdisciplinary team
• communicate with physicians and partner agencies
• conduct patient visits
• perform clinical pronouncements of death
• maintain accurate documentation
• supervise Home Health Aides.
