The Hospice Registered Nurse (RN) Case Manager role is onsite in North Charleston, SC, serving Charleston, Berkeley, and Dorchester Counties. Pay is $47 per hour. This is a 13-week, full-time contract supporting Interim HealthCare’s hospice team. The nurse manages an assigned hospice caseload and coordinates individualized care to support patient comfort, dignity, and quality of life. Assessments, care planning, coordination with the interdisciplinary group, and supervision of assigned care staff help meet changing patient and family needs.

What you'll do

• Manage an assigned hospice caseload
• complete patient and family assessments
• assess physical condition, pain, symptoms, and psychosocial needs
• develop and revise individualized plans of care with the interdisciplinary group, patient, family, and attending physician
• coordinate services and transitions
• supervise assigned LPNs/LVNs, hospice aides, and homemakers
• participate in required on-call duties
• and complete clinical documentation.
