What you'll do

• Evaluate patients and establish and assess treatment programs, plans of care, goals, and discharge plans in collaboration with physicians
• implement speech-language therapy plans and select activities to progress patients toward goals
• revise plans with patient or family input
• provide patient and family education
• provide discharge instructions, follow-up, and referrals as appropriate
