What you'll do

• Assist clients with applications for benefits and resources
• facilitate individual and group case management and intake orientation sessions
• offer make-up sessions for missed appointments
• complete assessments, reassessments, plan reviews, and daily documentation
• coordinate with program staff and external providers
• address social determinants of health and provide crisis or safety interventions as needed
• participate in audits, supervision, training, and quality reviews
• follow COMAR, CARF, Medicaid, licensing, and ethical requirements.
