What you'll do

• Conduct comprehensive patient assessments and develop and update individualized care plans
• provide skilled nursing care, pain and symptom management, medication administration, and patient education according to care plans and physician orders
• coordinate and supervise hospice aides and other healthcare professionals
• communicate with patients, families, physicians, and care-team members
• monitor physical, emotional, and spiritual well-being
• collaborate on care strategies aligned with patient wishes and goals
• ensure regulatory and documentation compliance
• provide bereavement support and resources as needed
• participate in team meetings.
